Molly published another artice (about a month ago) looking at 7 common mistakes.

  1. Document Encoding Problems (charset, UTF-8)
  2. Missing DOCTYPE Declarations
  3. Use of language Attribute in script Elements
  4. Missing type Attribute in style Elements
  5. img Element Errors
  6. Unnecessary Use of Proprietary Attributes
  7. Un-escaped Entities in XHTML

While I see all of them way too often, number 3 and 4 have been particularily evident lately.
